Support Coordination in Southeast Melbourne — Done the Human Way
Navigating the NDIS doesn't have to feel like a solo mission. Katelyn and Teri from the 360 Support Coordination team are here to walk alongside you — every step of the way.
Who We Are
We're Not Just Coordinators. We're in Your Corner.
Look, the NDIS is a brilliant system — but honestly? It can also feel incredibly overwhelming. There are plans to review, service providers to connect with, goals to map out, and paperwork that seems to multiply overnight. That's where we come in.
At 360 Support Coordination, we believe that great support coordination is less about ticking boxes and more about genuinely understanding what a good life looks like for you. Our Southeast Melbourne team — Katelyn and Teri — bring warmth, experience, and real commitment to every participant they work with.
Whether you're brand new to the NDIS or you've been a participant for years and just need a fresh start with better support, we'd love to have a conversation.
Why It Matters
The NDIS Can Open Doors — If You Have the Right Guide
The difference between a good NDIS plan and a great one often comes down to how well you understand what's available to you. A skilled support coordinator helps you find the right providers, avoid common pitfalls, make the most of your funding, and build real independence over time.
That's not a small thing. That's life-changing.

What We Do
Support Coordination That Actually Coordinates Things
Here's what Katelyn and Teri will do for you as your support coordinators in Southeast Melbourne. It's more than most people realise.
Understanding Your NDIS Plan
Your plan is written in funding categories and budget codes — not exactly bedtime reading. We'll break it down in plain English so you know exactly what you have to work with.
Finding the Right Providers
Southeast Melbourne has a lot of NDIS service providers. We know who's good, who's reliable, and who's going to be the right fit for you specifically — not just whoever has a vacancy.
Setting Up Service Agreements
We'll help you understand and set up your service agreements so everything is clear, fair, and working in your best interest from the start.
Goal Planning & Progress
Great support coordination keeps your NDIS goals front and centre — not just at plan review time. We check in regularly to make sure things are actually moving forward.
Crisis & Complex Support
When things get complicated — a hospital discharge, a breakdown in supports, a sudden change in circumstances — we're here to help you navigate it without losing your footing.
Plan Review Preparation
Plan reviews are high stakes. We help you prepare properly so that you go in with a clear picture of what's working, what needs to change, and what to ask for.
Community Connection
Building a life isn't just about disability supports. We help you connect with community, social, and mainstream services that complement your NDIS plan.
Building Your Independence
The long-term goal of support coordination is always to build your capacity and confidence to self-direct more of your own supports over time — on your terms.
Where We Work
Proudly Serving Southeast Melbourne
Katelyn and Teri are based locally and work across the Southeast Melbourne region — including suburbs across the inner southeast, outer southeast, and Mornington Peninsula corridor. If you're not sure whether we cover your area, just ask. Chances are, we do.
How It Works
What Happens When You Reach Out to Us
Getting started is a lot simpler than most people expect. Here's how our process typically works:
- 1
Have a No-Pressure Chat
We start with a relaxed conversation — no commitments, no forms to fill out. Just a chance for you to ask questions, tell us where you're at, and see if we feel like the right fit. Katelyn or Teri will take the call personally.
- 2
We Review Your NDIS Plan Together
Once you're ready to get started, we'll go through your current NDIS plan with you. We'll look at what funding you have for support coordination and map out what's possible within your plan.
- 3
We Create a Support Plan That Actually Fits You
Not a template. Not a copy-paste from another participant's file. A real plan built around your goals, your circumstances, and what a genuinely good life looks like for you.
- 4
We Connect You With the Right Supports
We identify and reach out to service providers on your behalf, help you compare options, set up your service agreements, and make sure everything starts smoothly.
- 5
We Stay With You
Support coordination isn't a one-and-done service. Katelyn and Teri check in regularly, monitor how your supports are going, and are always available when something comes up that needs attention.
Common Questions
Things People Often Ask Us
We get a lot of great questions — and we genuinely love answering them. Here are the ones we hear most often from people in Southeast Melbourne.
Support coordination is a funded NDIS support that helps you understand and implement your NDIS plan. A support coordinator (like Katelyn or Teri) helps you find and connect with service providers, set up your supports, build your capacity, and prepare for plan reviews.
Not everyone has support coordination in their plan, but many participants do — especially if they have complex support needs or are new to the NDIS. If you're not sure whether your plan includes it, we're happy to help you check.
You'll find it listed under the Capacity Building — Support Coordination category in your NDIS plan. It may also appear as Specialist Support Coordination if you have more complex needs. If you pull up your plan in the myplace portal and look for those terms, you'll see whether funding has been included. Can't find it or not sure how to read your plan? Just give us a call and we'll walk you through it.
Yes — absolutely. The NDIS is built on the principle of participant choice and control, and that very much applies to who coordinates your supports. You are not locked in to any particular organisation or individual. If you have support coordination funding in your plan, you can choose to work with 360 Support Coordination and have Katelyn or Teri as your coordinator.
These are two different roles and many participants have both. A plan manager handles the financial side of your plan — they pay your invoices and track your budgets. A support coordinator handles the practical and strategic side — finding the right providers, setting up your supports, helping with your goals, and being your go-to person when things get complicated. Katelyn and Teri are support coordinators, not plan managers, though we can certainly help you connect with a good plan manager if you need one.
If you have support coordination included in your NDIS plan, the cost comes directly from that allocated funding — not from your own money. It's part of what your plan is designed to pay for. The NDIS sets the pricing for support coordination through the NDIS Price Guide, so you know exactly what you're working with. During our initial chat, we'll be completely transparent about how we structure our services and what your funding covers.
Katelyn and Teri work across a broad area of Southeast Melbourne — including Frankston, Dandenong, Cranbourne, Berwick, Narre Warren, Pakenham, Noble Park, Springvale, Mornington, Glen Waverley, Oakleigh, Cheltenham, Clayton, Rowville, and surrounding suburbs. If you're not sure whether we cover your specific area, just get in touch — we'll do our best to accommodate you or point you in the right direction.
This is honestly one of the most common situations we see — and it's completely normal. NDIS plans can be confusing, especially when you're first starting out. That's precisely what Katelyn and Teri are here for. We'll sit down with you (virtually or in person), go through your plan from the beginning, explain what each line means, and help you understand what you can actually use your funding for. No question is too basic. We love starting from scratch with someone and watching everything click into place.
You have every right to change your support coordinator at any time. If your current coordinator isn't responsive, doesn't seem to understand your needs, or you just feel like it's not a good match — you're not obligated to stay. The process for switching is relatively straightforward, and if you reach out to us, we'll walk you through how to make the transition smoothly without disrupting your supports.
Yes. Katelyn and Teri have experience working with participants across a wide range of disabilities — including physical disabilities, intellectual disabilities, autism spectrum disorder, acquired brain injuries, psychosocial disabilities, and more. Every participant is different, and we approach each person's situation with fresh eyes and genuine curiosity about what support will work best for them.
Absolutely — in fact, preparing for your plan review is one of the most valuable things a support coordinator does. Katelyn and Teri will help you review what's worked and what hasn't over the course of your current plan, gather evidence and reports from your service providers, identify what needs to change or be added in your next plan, and help you feel confident and prepared going into your review meeting with the NDIA. A well-prepared plan review really can make a significant difference to the supports and funding you receive.
That depends a lot on your needs and preferences. Some participants like to check in weekly; others prefer monthly updates with availability for ad-hoc contact when something comes up. Katelyn and Teri will work with you to establish a contact rhythm that suits you. What we can promise is that you won't feel like you're chasing us — we're proactive about staying in touch and making sure things are on track.
Yes. 360 Support Coordination is a registered NDIS provider, which means we meet the quality and safeguarding requirements set by the NDIS Commission. This matters because it means you can trust that our services are delivered to a recognised standard and that your rights as a participant are protected.
We know that sometimes things don't wait for business hours. Katelyn and Teri both understand that crises and urgent situations are part of the reality for many NDIS participants, and they take that seriously. When you join the 360 team, we'll make sure you know exactly how to reach us when something urgent comes up — and we'll do our best to respond promptly and practically.
