Who Can Refer to Us
LACs & NDIA Planners
We work closely with LACs to ensure smooth transitions from planning to service delivery. We communicate clearly, respond promptly, and never leave a LAC wondering what happened to their referral.
Allied Health Professionals
OTs, psychologists, speech pathologists, and physiotherapists — we coordinate alongside your clinical work, not in competition with it. We respect existing therapeutic relationships and always communicate with your team.
Hospitals & Discharge Planners
We accept urgent referrals for participants being discharged from hospital who need rapid support coordination. Call us directly for time-sensitive referrals.

Best for: Participants needing moderate ongoing plan implementation support
Level 3 Specialist Support Coordination
Available in: Melbourne, Albury Wodonga, Perth
Best for: Complex cases, multiple providers, psychosocial presentations, crisis situations
Psychosocial Recovery Coaching
Available in: Melbourne, Albury Wodonga, Perth
Best for: Participants with psychosocial disability, mental health recovery goals, NDIS plan includes recovery coaching funding
Advanced Behaviour Support Practitioner
Available in: Western Suburbs Melbourne (David W), Northern Suburbs Melbourne (David W), South Eastern Suburbs Melbourne (Shana)
Best for: Participants requiring a behaviour support plan, functional behaviour assessment, or restrictive practice oversight
How to Refer — 3 Steps
Submit a Referral
Complete our online referral form or call +61 3 7047 6747. You need: participant name, NDIS number (if available), service needed, and your contact details. Nothing else is required to start.
We Contact You Within 24 Hours
A member of our team will contact you within one business day to confirm receipt and discuss the referral. For urgent referrals, call us directly.
We Keep You Updated
With participant consent, we provide referral partners with updates at key stages: initial contact made, service agreement signed, and services commenced. You are never left uninformed.
What We Ask of Referral Partners
Be transparent: Tell us honestly about the participant's situation, including any previous provider breakdowns, risk factors, or challenges. We can only deliver good outcomes with good information.
Confirm consent: Please ensure the participant or their guardian is aware of and consents to the referral before submitting.
Stay in touch: We welcome ongoing communication with referral partners. If something changes for the participant, let us know.
