If you want a support coordinator Melbourne can rely on, 360 SC is here. We don’t just process paperwork – we partner with you to turn your plan into real progress.

Support Coordination We Offer

The NDIS offers three levels of support coordination , and at 360 SC we specialize in the two most intensive levels:

Both levels are capacity-building, meaning we empower you to eventually manage more on your own. As the NDIS explains, coordinators help you “connect with NDIS providers, community, mainstream and other government services” and “build your confidence and skills to use and coordinate your supports”. With 360 Support Coordination, you can transition between levels if needed – for example, starting with specialist support and stepping down to standard coordination as goals are met. (Our friendly team will discuss this with you at your plan review.)

Benefits of a Local Melbourne Team

Choosing a Melbourne-based support coordination provider has big advantages. Our clients benefit from:

Our support coordinators know Melbourne inside and out. We understand Victoria’s NDIS rules and have extensive connections with local providers and agencies. This means we can quickly identify the right services nearby – from speech therapy in Glen Waverley to physiotherapy in Bendigo, we have contacts everywhere.

Melbourne has a diverse, well-connected disability community. As one local provider notes, “Melbourne is home to a wide range of services… with a coordinator, you can tap into local resources and community programs”. We leverage these resources – whether it’s local support groups, recreation programs, or specialist clubs – to enrich your plan beyond paid supports.

With offices in Bundoora and Moonee Ponds (and support coordinators around the city), we offer in-person meetings and quick on-the-ground help. Need an urgent meeting? Want a home visit? Our local presence means faster response times. We also understand Melbourne’s diverse cultures and language groups, making it easy to communicate in the way you prefer.

We know the Melbourne suburbs and services. For example, our coordinators can advise on public transport and mobility in Melbourne’s CBD or find local NDIS-approved home care providers in Geelong. Our local insight can save you time and stress.

By choosing 360 Support Coordination, you get the personal touch of a small local team and the expertise of a large provider. We combine the best of both: friendly neighbourhood support with professional NDIS knowledge.

Services Beyond Support Coordination

In addition to support coordination, 360 Support Coordination offers a range of related services to meet your NDIS needs:

For participants with psychosocial disabilities or mental health challenges. Our recovery coaches provide specialized coaching to help you manage daily living, build resilience, and achieve your recovery goals. This might include goal-setting around independent living, mental health education, or connecting with community mental health services.

We have qualified Behaviour Support Practitioners who work with you and your support team to understand and transform challenging behaviours. Our Behaviour Support Practitioners develop behavior support plans focused on positive strategies and building independence.

We don’t just help participants – we also train support coordinators. Our expert training programs build the skills of other coordinators and disability workers. This ensures the whole sector has qualified professionals. (If you’re a support worker seeking professional development, ask us about our workshops.)

In short, 360 Support Coordination is a one-stop shop for plan implementation: from coordination and coaching to clinical support and training. You can add these services to your plan at any time. For example, many participants combine support coordination with recovery coaching, and we’ll make it seamless since it’s all in-house.

Frequently Asked Questions

What is Support Coordination?

It's a capacity-building service funded by the NDIS. A support coordinator helps you use your NDIS plan effectively – explaining your budget, finding and coordinating providers, and connecting you to community supports. Essentially, a coordinator is your guide to the disability system. If you have questions about your plan or the options available, a support coordinator can help answer them.

Who can provide support coordination?

You may choose any registered NDIS provider of coordination services, or even an unregistered provider (if your plan allows). At 360 Support Coordination, we are a fully registered provider. You can switch to us at any time by issuing a "request for service" through the NDIS portal or simply calling us – your LAC or planner can also connect you.

What's the difference between Level 2 Support Coordination and Level 3 Support Coordination?

Level 2 (Coordination of Supports) is for participants needing standard help to navigate and implement their plan (e.g. most young adults, people starting out). Level 3 (Specialist Support Coordination) is for complex cases – for example if you have multiple disabilities, complex health needs, or extreme behaviors. We'll work with your planner to make sure you have the right level. If you're unsure, just talk to our team – we'll explain which level fits your situation.

How do I get support coordination in my plan?

During your NDIS planning meeting, you can request support coordination as part of your Capacity Building budget. If you already have an NDIS plan but no support coordination, you can speak to your LAC or planner about a plan review or reassessment. The NDIS says you are entitled to the level of coordination that is "reasonable and necessary" for your goals. Once your plan is approved with support coordination funding, you're free to choose our services.

Can I change support coordinators?

Absolutely. You have the right to change providers at any time. The NDIS says you can switch coordinators whenever you wish. If our approach isn't working for you, or if you simply want to try a different style, just tell us. We'll help you transfer your plan documents to the new coordinator with no fuss.

How can I find a support coordinator in Melbourne?

You can use the NDIS Provider Finder tool to search for providers by location, or ask for referrals from Local Area Coordinators (LACs) or community organisations. However, the simplest way is to contact us directly at 360 Support Coordination. We provide dedicated support coordination services tailored to your needs. Visit 360sc.com.au to learn more or get in touch with our team today.

What happens if my needs change?

We'll adjust! Your support plan is reviewed at least every 12 months. We meet with you to discuss progress and any new needs. If new needs arise mid-plan (e.g. after a health event), we can help prepare a plan variation request. With 360 Support Coordination, you have a partner who adapts with you, ensuring your NDIS plan always reflects your current goals.

What areas do you cover?

360 Support Coordination is based in Melbourne and covers all metropolitan and regional Victoria. We have offices in Bundoora (Uni Hill, Level 2) and Moonee Ponds (Hall St), so we're well-positioned to serve the northern and western suburbs. We also work remotely with clients statewide. No matter where you live in Greater Melbourne or surrounds, we can support you.
