What is an Incident?
An incident is any event or occurrence that:
- Results in harm, or has the potential to result in harm to a participant
- Involves a serious breach of the NDIS Code of Conduct
- Requires immediate action and/or remedial measures
- May be reportable to the NDIS Commission
All incidents must be reported as soon as possible after they occur or are discovered.
Reportable Incidents
The NDIS Quality and Safeguards Commission requires reporting of serious incidents including:
- Death of a participant
- Serious injury of a participant
- Abuse or neglect of a participant
- Unlawful sexual or physical contact with, or assault of, a participant
- Sexual misconduct committed against, or in the presence of, a participant
- Unauthorized use of restrictive practices
