The NDIS offers three levels of support coordination , and at 360 SC we specialize in the two most intensive levels:

We’ll phone you to learn about your needs, goals and situation. This call (or video meeting) is about understanding you – no pressure, just sharing information. It helps us match you with the right coordinator.
If you decide to proceed, we send out the consent forms, service agreement and any documentation. We handle this paperwork for you, explaining every detail so you know what to expect.
Your coordinator will do a thorough intake assessment. Together, you’ll review your NDIS plan in detail, clarify your goals, and identify any challenges. This ensures the support plan we build is perfectly tailored to you.
We assign you a dedicated Melbourne-based coordinator. They’ll schedule your first face-to-face meeting to kick off services. From here on, your coordinator will guide you through all aspects of implementing your plan.

Level 2 (Coordination of Supports) is for participants needing standard help to navigate and implement their plan (e.g. most young adults, people starting out). Level 3 (Specialist Support Coordination) is for complex cases – for example if you have multiple disabilities, complex health needs, or extreme behaviors. We'll work with your planner to make sure you have the right level. If you're unsure, just talk to our team – we'll explain which level fits your situation.
During your NDIS planning meeting, you can request support coordination as part of your Capacity Building budget. If you already have an NDIS plan but no support coordination, you can speak to your LAC or planner about a plan review or reassessment. The NDIS says you are entitled to the level of coordination that is "reasonable and necessary" for your goals. Once your plan is approved with support coordination funding, you're free to choose our services.
Absolutely. You have the right to change providers at any time. The NDIS says you can switch coordinators whenever you wish. If our approach isn't working for you, or if you simply want to try a different style, just tell us. We'll help you transfer your plan documents to the new coordinator with no fuss.

We'll adjust! Your support plan is reviewed at least every 12 months. We meet with you to discuss progress and any new needs. If new needs arise mid-plan (e.g. after a health event), we can help prepare a plan variation request.
