how we work with ndis participants
Our Approach with Participants
Step 1 – Referral or Contact
Reach out via our enquiry form, phone, or email to express your interest in our Support Coordination service. Or, ask your LAC to send us a referral.
Step 2 – Initial Consultation
We call you (or respond to your email) to gather preliminary information about your needs and goals. This helps us understand how best to support you.
Step 3 – Documentation
If you would like more information or would like to proceed, we will send you the necessary documents, including consent forms, service agreements, and any other relevant paperwork.
Step 4 – Assessment
We’ll conduct a thorough intake assessment to tailor our services to your unique situation.
Step 5 – Support Coordination Begins
Your Support Coordinator will be allocated and will reach out to schedule your first meeting.
Step 6 – Review Every 6-8 Weeks
Your Support Coordinator will sit with you every 6-8 weeks to advise about the progress of your plan and get a feedback from you.
